Build a provider index
List each provider, facility, date range, and the reason for the visit in plain terms. This index helps locate records; it should not diagnose an injury or replace the provider’s documentation.
Keep a treatment timeline
Record appointment dates, referrals, written restrictions, and missed appointments. If you keep symptom notes, label them as personal observations and avoid rewriting them later from memory.
Separate charges, payments, and balances
A bill, an insurance explanation of benefits, and a receipt answer different questions. Keep all three when available and do not treat a billed amount as proof of the final amount owed.
Share records carefully
Confirm the recipient and use a secure intake method before sending medical information. Ask what an authorization permits, how long it lasts, and whether it can be narrowed or revoked under applicable law.
